Student progress forms below are necessary for submission to the Graduate School of Biomedical Sciences and can be downloaded and filled out by the student in preparation for major events in the Radiological Sciences graduate degree granting process.

Steps to Degree Approval at Graduate Faculty Council
Submit the following items at least 5 days prior to the next GFC meeting which is scheduled on the second Friday of each month. If you have any questions, contact your program administrator or Janice Stong in the Graduate Dean’s office.
- Form 40 - Request for Final Defense and Oral Examination
- Form 41 - MS Report on Final Oral Examination
- Form 42 - Thesis Proposal Committee Composition Approval
- Form 43 - PhD Report on Final Oral Examination
ProQuest/UMI Submission
The Proquest/UMI website provides publishing guides regarding your options for traditional publishing verses open access publishing and registering for copyrights. There are fees associated with some of these additional options. Beginning September 27, 2010, there will be no fee for traditional publishing using ProQuest UMI; however, there will be a required $25.00 fee for File Transfer Delivery to the UTHSCSA library for both dissertations and thesis.

[edit] Radiological Sciences Graduate Program Forms
Students may petition the Radiological Sciences COGS to approve a change of faculty advisor after both the current advisor and new advisor have documented their agreement by signing this form.
Students must fill out this form and with supporting documentation (acceptance of abstract, announcement of travel award, etc.) at least six weeks before taking a University-related trip
Students and faculty advisors are required to meet once a year to complete this form
This is to be completed, signed and returned to the Radiological Sciences Graduate Office for processing. The University no longer requires a separate clearance form. It is recommended that you begin the clearance process once you have completed your Oral Defense of Dissertation/ Thesis.
New faculty need to complete this form to meet Graduate School accreditation guidelines.
FORMS FOR REQUESTING WAIVERS OF COURSEWORK
- Form GSPRSW01 - Request for waiver based on similar course completed at another institution.
- Form GSPRSW02 - Request for waiver of 1 hour of credit for RADI 5090 based on a scientific presentation at an international, national or regional meeting.
- Form GSPRSW03 - Request of waiver of first year requirements for Human Imaging coursework based on equivalent coursework completed in medical school.
